Your application MUST include all of the following or it will be returned:
_____ Check for $150.00 payable to LVFA.
_____ Copy of State of California State Board of Equalization Sellers Permit. (The person attending the Festival must have their name on the Sellers Permit.)
_____ A current Certificate of Insurance for $1,000,000 general liability naming LVFA and the City of Lompoc as additional insured.
_____ Four photos of your merchandise.
_____ City Permit. Call (805) 875-8242 for your fees or visit the City website at www.cityoflompoc.com (Handcrafted items are exempt if sales are under $2625.00 annually.) Please read attached letter regarding the City permit. Do NOT send the City Permit application to the Lompoc Valley Festival Association; Send it to the City of Lompoc, or it will be returned to you. We require a copy of the City Permit with your application.
Complete Applications are due in the Festival Office by Friday April 6, 2012
I understand that I assume all risks for loss, theft, damage, liability, injury, cost or expense that I may incur during the 2012 Spring Arts Festival, and further agree that I will save, defend and hold harmless the Lompoc Valley Festival Association and the City of Lompoc and their directors, officials, volunteers, employees and agents from any claims, loss liability, damage or cause of action, including attorneys’ fees, that I may incur, suffer or be required to pay in the course of my participation as an exhibitor in the Festival. I further assume full responsibility for the safety and insurance of my own exhibit, for all sales tax occasioned by its use, and for picking up any and all trash in the area assigned to me. I further understand and agree to the following rules:
• To display copies of my Seller’s Permit, and City of Lompoc Permit at my exhibit space.
• To sell only those items listed on my application
• To conform to all entry and exit traffic patterns on event days.
• To allow NO PETS or loud audio equipment within or around my assigned space.
• To not exceed my assigned space size.
• To not infringe on walkways or neighboring booth spaces, and not move to another location without official permission.
I also understand that refunds will not be granted on any funds received after April 1st, 2011. Entry fees are nontransferable without written Festival authorization. I certify the information on my application is true and correct and that I have read the above rules and understand and agree to abide by them.
